COACHES SELECTION PROCESS
Each year, CYLA has a process of selecting Head & Assistant Coaches for the Spring season. All interested coaches are required to submit an application for consideration. All
applications are reviewed by the Coaches Selection Committee, which
consists of the Athletic Director and 3 CYLA parents.
Please complete the following
steps:

COACHES BACKGROUND CHECKCYLA requires all
coaches to go through a background check. To register go to ePATCH.
Run through the application and print the findings - print the
Certification Form. This is generated after the Control Number is
returned. Careful - don’t stop until you get to an official
page that has a seal in the background. For instructions click: (Background Check Instructions). CYLA will reimburse the $10 cost
after receipt of “response fro criminal record check”. Submit a copy to John Colt and TJ Antich for
payment.
BOYS COACHES - U.S. Lacrosse Membership Reimbursement
All CYLA coaches must be U.S. Lacrosse members!!!
If
you haven't signed up and paid your 2010 dues already, please do so.
CYLA reimburses coaches their $50 dues fee upon presentation of receipt
and USL membership number.

If you ARE a CYLA coach, and have NOT submitted your receipt, please email your receipt to CYLA Treasurer at TJ Antich. It will be easiest to write ALL reimbursement checks at once rather than one-at-a-time.
